What is Percutaneous Nephrolithotomy (PCNL)?
Percutaneous Nephrolithotomy (PCNL) is a minimally invasive surgical technique used for treating kidney stones. The word “percutaneous” means “through the skin”; thus, this procedure is performed by accessing the kidney through a small incision in the skin, instead of an open surgery. PCNL is primarily used to fragment and remove kidney stones that are large, hard, or complex.
How is PCNL Performed?
The procedure is usually performed under general anesthesia.
- A small incision, approximately 1 cm in size, is made in the patient’s back.
- A thin channel is created through this incision to the kidney, and special instruments are guided into the kidney.
- The stones are broken up using laser or ultrasonic devices.
- The fragmented stones are separated into small pieces and removed through the channel.
- A temporary catheter (nephrostomy tube) may sometimes be placed at the end of the procedure to ensure the urinary tract remains open.
In Which Cases is PCNL Preferred?
PCNL is the preferred treatment option for:
- Large kidney stones measuring more than 2 cm.
- Complex or multiple stones occurring together.
- Stones that cannot be treated by ESWL (shock wave lithotripsy) or other endoscopic methods.
- Kidney stones that are anatomically challenging to access.
Advantages of PCNL Treatment
- It is less invasive than open surgery, meaning the incision is small and recovery is faster.
- Provides a high success rate in clearing large stones.
- The length of hospital stay is shorter.
- The recovery process is more comfortable.
Potential Risks and Complications
As with any surgical procedure, PCNL carries certain risks:
- Bleeding (mostly minimal and resolves spontaneously).
- Infection.
- Injury to the kidney or surrounding tissues.
- Urine leakage.
- In rare cases, the use of a nephrostomy tube may be necessary.
Recovery Process
- Patients typically remain in the hospital for a few days after the procedure.
- Recovery is fast, and the return to normal life usually occurs within a few weeks.
- Pain and discomfort can be managed with pain relievers recommended by the doctor.
- Follow-up appointments and imaging tests are used to monitor whether the stone has been completely cleared.
Percutaneous Nephrolithotomy is a highly effective and reliable method for treating large and complex kidney stones. Applied with modern medical technology and focusing on patient comfort, this method ensures rapid and successful removal of stones.
